Med-ID for First Responders

Med-ID for first responders is designed to provide an easily accessible method for first responders to access important medical information upon their arrival to a scene. The Med-ID form available below consists of an informational card that you fill out with pertinent medical information. After filling out the form, pick up a brightly colored magnetic pouch from the police department to hold your form and place it in a highly visible area of your home.
